Síndrome de Gilbert

Algunas personas tienden a presentar en forma intermitente coloración amarilla de las escleras (la parte blanca del ojo) o de la piel sin tener otros síntomas de enfermedades del hígado. La gran mayoría de estas personas tienen lo que se llama síndrome o enfermedad de Gilbert. Esta es una afección benigna muy frecuente (aproximadamente el 9% de la población está afectada).

Algunas situaciones como el ayuno prolongado, falta de sueño, una infección, stress o cirugías pueden hacer que el nivel de bilirrubina aumente más. Es importante reconocer esta afección para evitar exámenes y estudios innecesarios.

La situación más habitual que lleva al diagnóstico es el hallazgo de bilirrubina elevada en exámenes (p. ej. perfil bioquímico) realizados por rutina o por síntomas no relacionados con el hígado. Esto hallazgo muchas veces genera ansiedad en el paciente y, lo que es peor, puede llevar al médico no familiarizado con esta afección a realizar una serie de exámenes que no son necesarios.

En los exámenes de sangre lo central es que la bilirrubina total está aumentada, pero la bilirrubina conjugada (también llamada “directa”) está en niveles normales. Raramente la bilirrubina total supera los 5 mg/dL. Los otras exámenes hepáticos habituales como transaminasas (SGOT y SGPT), fosfatasas alcalinas (FA), gama glutamil transpeptidasa (GGT), albúmina y tiempo de protrombina están dentro de límites normales. Normalmente se pide además un hemograma para descartar hemólisis (enfermedades caracterizadas por destrucción acelerada de los glóbulos rojos), ya que estas enfermedades pueden dar un patrón similar de elevación de la bilirrubina.

El síndrome de Gilbert se origina en una disminución de la capacidad de excretar bilirrubina por la célula hepática. La bilirrubina es producto de la degradación de la hemoglobina presente en los glóbulos rojos. Normalmente la bilirrubina es captada por hígado y se “conjuga” (une) a ácido glucurónico, lo que permite que sea soluble en agua y pueda ser excretada en la bilis. La enzima que conjuga la bilirrubina se llama uridindifosfoglucuronato glucuronosiltransferasa (UGT) y su producción se regula por un promotor que puede tener una mutación que provoca una menor producción de esta enzima. El síndrome de Gilbert se hereda de forma autosómica recesiva.

No se requiere ningún tratamiento específico para la enfermedad o síndrome de Gilbert.

This message was not composed either to accuse or to absolve, but solely written to shed some light on the recent incidents, resolving in the cancellation of two Ordo Rosarius Equilibrio concerts i Lisbon and Gaia Portugal on the weekend of October 14th and 15th 2005.

Ordo Rosarius Equilibrio and Spiritual Front were recently invited by Sol Negro Production / Sol Pedra Magazine to perform in Lisbon and Porto Portugal on the weekend of October 14th & 16th 2005. It was all agreed and about to happen; but we only came as far as the airport before we realized that a series of unexpected (organizational and administrative) complications had put an end to our plans, complications which ultimately resolved in an even more unexpected cancellation of both concerts at 16.00 (04.00 p.m) yesterday afternoon.

After more or less eight hours at Arlanda airport Stockholm Sweden, from 05.00 – 12.30, it became unpleasantly clear that all attempts to get to Porto Portugal, were in vain; the tickets remained booked but also unpaid, and when the KLM personnel kindly informed me over the phone, that our reservation had been cancelled from Portugal at 14.30 (02.30 p.m), it was only a matter of time before another phone call from Sol Negro Productions informed me that the whole event had been cancelled.

It is difficult to determine who is ultimately responsible for all this, for the mistakes which ultimately resulted in the concerts being cancelled, so I will not use this opportunity to speculate and make accusations, but solely extend my apologies to each and everyone who had hoped to see us play, who had bought tickets and possibly travelled quite some distance to attend the concerts. We are truly sorry on your behalf, and hope that the possibility of returning to Portugal and perform, will present itself again.
